• Returning non-MCNHS degree-seeking students who want to declare a new major within MCNHS. Please note you must be in 良好的学术地位 to use this form.
• Former MCNHS majors who want to return after being out for 5 or more years. Student in MCHNS who have taken fewer than 5 years off, please use the General Re-Enrollment Application

 Accelerated Master's Program

  • b.  Honors College students with at least 50 completed credits.
  • c.  Transfer students with at least 30 in-residence completed credits.
• Current students who wish to remove AMP interest from their undergraduate record
  • This does not update 研究生 招生 records if you have applied

College of Management Form 

• Non-Management students who want to declare a Management 小 or a major within CM – Management or IT (CM)
• Current Management students who want to switch majors within CM – Management or IT (CM)

College of Management Concentration/Track Add/Change form

• Only for current degree-seeking Management students who want to declare or add an additional concentration or track.

• Non-Nursing students who want to declare a major within MCNHS
• Current MCNHS students who want to add or switch programs within the MCNHS
